Identification Requirements:

• One form of identification must be photo identification – such as a driver’s license, DMV ID card or a passport.
• A second form of identification can be a registration card, military card, voter registration card, employer identification card or school identification.

Form DCP-115 must be completed in duplicate by the requesting person and agency if applicable.
